Art QR Codes

QR codes by use Surveys

QR codes for surveys and questionnaires

People answer a survey when it is right in front of them: on the receipt, the table, the exit door. A code opens it on their phone in one scan.

Free and static: paste your link, download SVG or PNG, no sign-up, never expires. Want art, a link you can change later and scan counts? Make it an art code.

Your code appears here as you type.

QR codes for surveys: the facts

What the code holds
A link to the survey: a Google Form (forms.gle), SurveyMonkey, Typeform or Microsoft Forms. A pre-filled link can also record where the code was, for example the store number.
Print size
1 in on a receipt, 2 in on a table card or counter sign, 4 in on a door or exit sign.
Scan distance
Receipts and table cards are scanned in hand; exit signs from 3 to 5 ft.
What breaks it
Long pre-filled links. A Google Forms pre-filled address can be 150 characters or more, which makes a dense code that fails small. Use a dynamic code: the printed code stays short whatever the link.
Good to know
Phone surveys lose people after a couple of minutes. Five questions or fewer, and a sign-in-free form, get far more answers. Google Forms that collect "verified" emails make people sign in to Google first.

Make it art, tested to scan

We paint the code into a picture in your colours, then test it with three decoders under 16 print and phone conditions before you can download it.

Your codes outlive your subscription

Cancel and dynamic codes keep pointing at your last link. Read the promise.

Questions

How do I make a QR code for a Google Form?
In the form press Send, choose the link tab, tick "Shorten URL" and copy the forms.gle link. Paste it into the generator above.
Can one survey tell me which location the answer came from?
Yes. Use a pre-filled link with the location already filled in (in Google Forms: the three dots, then "Get pre-filled link"), and give each location its own code.
How many questions should a QR survey have?
Five or fewer for people on a phone, with the most important question first. Long surveys get abandoned halfway.
Can I reuse the printed code for next quarter's survey?
With a dynamic code, yes: point it at the new form and the same signs keep collecting answers.
