This template

Colours
Dots #2b2f73, corners #1b1e4f, background #fdfdff.
Contrast
11.8:1 between the dots and the background. Above 4.5:1 reads reliably in dim rooms; plain black on white is 21:1.
Shapes
Rounded dots and soft corners. The corner squares keep a crisp outer edge, which phone scanners use to find the code.
Frame
No frame: the plainest option, best for small prints and long distances.
Extras
A linear gradient on the dots: #2b2f73 to #2b5d73. Each corner square has its own colour: #2b2f73, #2b5d73 and #552b73. A office building icon in the centre (level H error correction keeps it scannable).
Palette
Policy indigo: see every style in this palette.
Good for
Insurance agents, advisors and claims desks.
