This template

Colours: Dots #174b49, corners #174b49, background #f6ffff.
Contrast: 9.7:1 between the dots and the background. Above 4.5:1 reads reliably in dim rooms; plain black on white is 21:1.
Shapes: Square dots and square corners. The corner squares keep a crisp outer edge, which phone scanners use to find the code.
Frame: Framed card reading "Ask For Help". A short instruction next to a code gets more scans than a code alone.
Extras: 35% of the dots in a second colour, #184e2a.
Palette: Teal: see every style in this palette.
Good for: Service desks and shops.
