This template

Colours: Dots #4b3223, corners #4b3223, background #fffaf4.
Contrast: 11.4:1 between the dots and the background. Above 4.5:1 reads reliably in dim rooms; plain black on white is 21:1.
Shapes: Dots dots and soft corners. The corner squares keep a crisp outer edge, which phone scanners use to find the code.
Frame: Label below reading "Read The Story". A short instruction next to a code gets more scans than a code alone.
Extras: 20% of the dots in a second colour, #4c4c22.
Palette: Umber: see every style in this palette.
Good for: Makers, furniture and craft brands.
