This template

Colours: Dots #0e5559, corners #09393c, background #f5fbfb.
Contrast: 8.2:1 between the dots and the background. Above 4.5:1 reads reliably in dim rooms; plain black on white is 21:1.
Shapes: Pill dots and soft corners. The corner squares keep a crisp outer edge, which phone scanners use to find the code.
Frame: No frame: the plainest option, best for small prints and long distances.
Extras: The background fades from #f5fbfb to #d0eff1. Rounded corners on the whole code, like a sticker.
Palette: Clinic teal: see every style in this palette.
Good for: Clinics, dentists and front desks.
